Vaccine Clinic (RSV & Flu) Shots

Today is the last day to complete your medical forms for the RSV and FLU shots provided by Edwards Pharmacy. 

All paperwork has to be delivered to Edwards by tomorrow morning (October 1st). 

Please deliver completed forms to the Clubhouse, in the brown box provided in the Wall Street Room no later than 6:00 pm tonight.
