The Lifestyle Committee, in conjunction with Edwards Pharmacy, is bringing flu shots to SV!
 Sign up sheet is in the Wall Street Room.
Times are every 10 minutes on Thursday, October 6th from 

9 am-12 pm

and then later that afternoon from

3 pm-6 pm.
An Edwards Pharmacy Pharmacist will be here administering vaccines.

Please sign up in a specific time slot and take and fill out an informed consent to bring back with you to the clinic.

There will be a sign-up sheet in the clubhouse Wallstreet Room, along with a copy of the paperwork for Edwards Pharmacy. In order to expedite the process, please have this paperwork filled out and bring it with you when you come for your shot. Having the paperwork completed ahead of time will be a time-saver and enable Edwards to serve more residents. Also, remember to bring your insurance card and your Medicare card if you have one.

Please note that Edwards Pharmacy is CPR certified and an AED will be available in the event of an adverse reaction to the vaccine.
